The growing demand for proper monitoring and treatment of chronic disease patients is unsustainable for European healthcare systems. Despite the recent advancements, remote patient monitoring (RPM) systems have not achieved their potential due to their inability to integrate accurate data from any given medical device to the Electronic Health Records (EHRs).
